Died on 11th December at Mount Vale Care Home, Northallerton, aged 74 years.
Beloved wife of Paul, mum of Philip and Jenny and friend to many.
A celebration of her life will take place on Thursday 8th January at Maple Park Crematorium at 10am.
Family flowers only please, donations received in memory of Hazel will be for the Residents Fund, Mount Vale Care Home.
