Tracey Ann Hammond
Sadly passed away on the 3rd of July, aged 48.
Much loved daughter, sister, mother, auntie and a dear friend to all who knew her.
Funeral service will take place at Kingsdown Crematorium on Tuesday the 22nd of July at 11.15am.
The family would like everyone to wear bright colourful clothes for Tracey's funeral.
Family flowers only. Donations in memory of Tracey to be made online to the Prospect Hospice.
